According to BS guidelines, surgical instruments must bear the name or registered trademark of the manufacturer or supplier. While this may seem like a minor violation, it holds paramount importance. In the event of an instrument failure during service, notifying the supplier and manufacturer becomes crucial for addressing potential problems, ensuring patient and staff safety, and addressing liability and insurance concerns.

Mechanical and structural faults, particularly machining burr debris, were also common. BS mandates that all surfaces must be free from pores, crevices, and grinding marks. The intricate metallic surfaces of surgical instruments undergo various engineering processes, and any imperfections may lead to problems. Incomplete processes can result in blood and tissue debris accumulation, raising concerns about the effectiveness of sterilization procedures. Additionally, metallic fragments could pose risks by remaining as microscopic debris in wounds.

Cracks and soldering faults were identified as potential issues, providing niches for the retention of blood and tissue, and in severe cases, leading to instrument failure. Scissors that do not cut and artery clips with ratchet problems were also noted, highlighting the importance of adhering to BS requirements for clean cutting and proper functioning of instrument racks.

Fine tissue forceps with guide pins were found to have issues where the guide pins protruded on light closure, possibly contributing to glove puncture. Even the stainless steel alloy, despite being subjected to corrosion resistance tests according to BS, surprisingly showed visible corrosion on new instruments.

In conclusion, this study underscores the significance of local quality control for new surgical instruments. While specific instances of harm were not demonstrated, the identified defects prompted supplier notification and remedial actions, emphasizing the importance of thorough inspections and adherence to standards in maintaining the quality and safety of surgical instruments.
